Specialist will work in coordination with the Niagara County Department of Mental Health Hygiene Practitioner to engage with individuals who are at risk for emergency room presentation and hospitalization and support their linkage to ongoing recovery supports. The Peer Specialist will provide a team response, meeting with the individual along with the NCDMH Clinician; will work with individuals who are in crisis respite (Hope House) to identify community support services and will provide direct support (through warm handoffs) to ensure linkages to those supports; will provide timely therapeutic interventions offering both individual interactions with both the peer specialist and NCDMH Clinician as well as providing supportive group activities coordinated by the Peer Specialist.

A mental health diagnosis and active, ongoing engagement in treatment for such is required of this position.
